Twin Lakes resident graduates from nursing rehab – but stays in program to keep strong

The program has helped build resident Phyllis's independence and confidence, team member says

A year ago, Twin Lakes Terrace resident Phyllis was having mobility challenges. Now, after participating in nursing rehabilitation at the Sarnia long-term care community, she’s regained her strength – and graduated from the rehab program.

But because the program has been so beneficial to her recovery, Phyllis has decided to continue working with the nursing rehab team in order to maintain her strength, says nursing rehabilitation co-ordinator Danielle Rogers.

Phyllis entered the nursing rehab program in March 2024. At the time, she wanted to improve her strength so she could do all her activities of daily living without assistance.

Phyllis focused on the dressing-and-grooming program and a one-to-one ambulation program with the nursing rehab team, and made great strides, Danielle says.

“Phyllis worked diligently with the nursing rehab team on a daily basis to meet her established goals and was determined not to give up,” Danielle tells S&R Today.

On Jan. 21, Phyllis reached her nursing rehab program goals. Her ambulation program goal was to walk 100 metres per day within a three-month time frame with one-person assistance using gait belt and walker. Her dressing-and-grooming program goal was to improve her ability to participate in her activities of daily living in three months' time.

Following Phyllis graduation from the nursing rehab program, Danielle met with the physiotherapy team to complete a follow-up mobility assessment.

“Phyllis was assessed and able to ambulate safely and consistently using a walker with only supervision from staff,” Danielle says.

At Phyllis's request, she continues to participate in an active range-of-motion program every day as a part of her restorative program.

“Phyllis has expressed that the nursing rehab programs have helped her improve her independence and confidence, and she is happy to be moving around the home more independently and plans to go for outdoor walks in the home's backyard on warmer days,” Danielle says.
